Forms to be used in contacts between KDPW_CCP and participants.

All forms should be submitted in Polish. For the original forms, see below: 

Form 1 – Application for a participation agreement and clearing member status with KDPW_CCP
Form 2 – Application for a participation agreement and non-clearing member status with KDPW_CCP 
Form 3 – Specimen signatures card 
Form 4 – Declaration giving consent to arbitration by the Arbitration Court  
Form 5 – Participant information card
Form 6 – Participant’s account number
Form 6a – Power of attorney for the payer
Form 6b – Power of attorney for the payer – foreign currency
Form 7 – Declaration indicating the Settlement Agent
Form 8 – Declaration of a Clearing Member on the cash or derivatives market organised by the WSE or the market organised by BondSpot SA
Form 9 – Declaration of a Clearing Member for transactions concluded in the alternative trading system organised by the WSE
Form 10 – Declaration of a Clearing Member for transactions concluded in the alternative trading system organised by BondSpot SA
Form 11 – Declaration of joining the ATS guarantee fund – WSE
Form 12 – Declaration of joining the ATS guarantee fund – BondSpot SA
Form 13 – Power of attorney for KDPW_CCP to close positions 
Form 14 – Power of attorney to submit orders
Form 15 – Declaration of joining the automatic securities lending system as borrower
Form 18 – Declaration of clearing member relating to posting of securities as maintenance margin, according to the rules for transfer of title
Form 19 – Declaration of an entity represented by a participant filed with KDPW_CCP S.A. (organised trading)
Form 20 – Return of cash deposited as Initial Margin
Form 21 – Return of cash deposited as Initial Margin (OTC)
Form 22 – Participant’s declaration designating to KDPW_CCP S.A. the identifier (ID) of the relevant account (default account in KDPW_CCP)
Form 23 – Designation / deletion / addition to KDPW_CCP of allowed account IDs
